Our vision discount card is provider by Trustmark, through Best Benefits. You may be eligible to receive a discount on exams and surgical procedures, including LASIK. Receive discounts up to 50% on eyeglasses, contact lenses (excluding disposable), non-prescription sunglasses, and other retail eyewear items through a network of providers. In our area, providers include Sears and Nationwide Vision. Best Benefits Vision Plan
The Best Benefits Vision program provides members and their families with significant savings on the purchase of corrective eyewear, contact lenses and related eyewear products. Membership entitles eligible participants to valuable savings from the first day of membership, everyday, all year.

This plan guarantees satisfaction and the lowest price. If a member finds a lower price anywhere else on the exact same pair of prescription eyeglasses purchased at a participating location within 30 days, the difference will be refunded.

If for any reason you are not happy with a purchase at the retail locations or through the mail, return the merchandise within 30 days for an exchange or full refund.

Ameritas Dental Insurance

Employees can go to any dentist of their choice for services.

The Town plan does not have a dental network. The Town pays the full cost of employee coverage. The Town pays 51.45% of the cost of dependent coverage. The remaining 48.55% is the responsibility of the employee. The cost is $20.84 per month and is a fixed rate regardless of the number of dependents added to the plan. $10.42 is deducted from each of the employee’s first two paychecks of the month. This rate became effective July 1, 2004; however, the rate is subject to change during our annual benefit renewal process.

ICMA Retirement Corporation

The Town does not participate in Social Security. This 401(a) plan is in lieu of Social Security contributions. Medicare is still withheld from the employee’s paycheck and matched by the Town.

Employee is required to contribute 5% of gross pay to this plan, and the Town contributes 7% into the plan on behalf of the employee. The employee determines where the money is invested based on choices offered by ICMA Retirement Corporation. The employee is always 100% vested in his or her own contributions. The vesting schedule for the Town’s 7% contribution is 20% after 1 year; 40% after 2 years; 60% after 3 years; 80% after 4 years; and 100% after 5 years of service.

Mandatory participation. All full-time employees who are not in the Public Safety Retirement Program are eligible upon hire.

The Town is currently evaluating enhancements to the retirement plan.

Public Safety Personnel Retirement System

UNUM Short-Term & Long-Term Disability

Short-Term Disability (STD)

Coverage provided by UNUM Life Insurance Company. All employees scheduled to work 30 hours or more are eligible on the 1st of the month following 30 days of employment.

Employees are eligible to receive benefits beginning with the 15th day of continuous disability due to an injury or a sickness. If you meet the definition of disability, you would be eligible to receive a weekly benefit equal to 50% of your basic weekly earnings, not to exceed the maximum weekly benefits. If you meet the definition of disability you may receive a benefit for 11 weeks.

There is no cost to the employee for this coverage; it is fully paid by the Town.
 

Long-Term Disability (LTD)

Coverage provided by UNUM Life Insurance Company. All employees scheduled to work 30 hours or more are eligible on the 1st of the month following 30 days of employment.

Before benefits are payable, employee must be totally disabled and unable to work due to an injury or a sickness for the 90 day elimination period or the end of accumulated sick leave, whichever is later. If you meet the definition of disability, you would be eligible to receive a benefit equal to 60% of basic monthly earnings, not to exceed the maximum monthly benefits, less other income benefits.

There is no cost to the employee for this coverage; it is fully paid by the Town.
